Terms of Reference (ToR)
Purpose
The Hybrid Stroke Rehabilitation Hub steering group will oversee the developments of both the pan-Lothian digital rehabilitation community and the physical locations.
Term
These terms of reference will apply from 1 February 2024 until the review date of 31 March 2025 and can be extended as required subject to the review.
Role of the steering group
The steering group aims to:
• Oversee the development of the hybrid stroke rehabilitation hub in Lothian
• Provide strategic direction to the digital and physical rehabilitation community development
• Establish and oversee two working groups to develop: 1. Digital community, 2. Physical hubs
• Oversee the partnership working between people with lived experience of stroke, third sector partners and health and social care partnerships
• Apply data considerations for informing the development of the stroke rehabilitation hub and to the collection of routine data for quality improvement and research
Chairing and Membership
The Stroke Rehabilitation Hub steering group will be co-chaired by Simon Harris and Charlie Chung

Prior to forming of the group, contact will be made with individual services to ensure that their involvement is indicated.
Members of the Stroke Rehabilitation Hub steering group will endeavour to work considerately and respectfully within and out with meetings. They should respect diversity, different roles and boundaries, and avoid giving offence.
The group will review membership and co-opt members as required.
Frequency of meetings
Monthly, with actions determined and deadlines set before the following meeting.
Programme governance
This steering group will report to Lothian Stroke Managed Clinical Network
It is important that operational management groups across Lothian are updated and have opportunity to discuss and inform emerging recommendations ahead of final recommendations.
A timeline of discussion and decision making will be determined.
Proposed Hub service
Physical Hubs
Buildings: locations include public spaces such as community centres, libraries and community cafes where there is existing footfall and a space for social interaction out with the location where scheduled sessions take place.
Services
• Scheduled drop in sessions with a focus on rehabilitation, These may include both general and specific sessions, e.g. hand and arm rehabilitation, cognitive rehabilitation
• CHSS supported self-management programmes
• Fatigue management programmes
• Individual sessions where community team members can meet with stroke survivors for rehabilitation service delivery
Technology based movement and cognitive rehabilitation using equipment available at Edinburgh Community Stroke Service, the Centre for Co-Creation, University of Strathclyde and the Lanarkshire Stroke Hub
Digital Community
• Online community where members can engage in conversations about their rehabilitation goals and activities
• Peer support and encouragement for peoples’ self-managed rehabilitation will be a focus
• Types of data can be explored in this community to inform individual rehabilitation programmes
• Professional engagement with rehabilitation professionals will be available for self-management coaching and signposting
